Occupational Therapist

Job Summary
Community Therapists is seeking a skilled Occupational Therapist to join our team in a community-based practice. As an OT, you will work with clients across the rehabilitation continuum, providing hospital discharge planning, cognitive assessments, and rehabilitation to increase client independence.
Key Responsibilities
- Develop and implement community integration plans to support clients in their recovery
- Collaborate with healthcare teams to ensure seamless care transitions
- Provide case management to facilitate client access to other health services
- Supervise rehab assistants and develop plans to increase home/community functioning
- Liaise with funders, family, and healthcare teams to ensure client needs are met
Requirements
- Member of COTBC or eligible for registration
- Driver's license and access to a reliable vehicle
- Excellent verbal and written communication skills
- Aptitude for negotiation and consensus building
- Experience working with insurance fee payers is an asset
- Experience working with clients with TBI is an asset
