Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Fraser Health. As a key member of our healthcare team, you will provide clinical pharmacy services, including individualized patient medication therapy, patient counseling, and consultation with healthcare providers to develop and discuss therapeutic plans.

Key Responsibilities
  • Provide clinical pharmacy services, including individualized patient medication therapy, patient counseling, and consultation with healthcare providers to develop and discuss therapeutic plans.
  • Assist the healthcare team in developing drug therapy guidelines for individual disease conditions by reviewing current literature, compiling and analyzing information, and providing suggestions and recommendations.
  • Participate in quality improvement activities, including reviewing current practices, performing quality assurance audits, and identifying actual or potential problems and recommending changes.
  • Perform drug distribution duties, including interpreting medication orders/prescriptions, dispensing medications, and ensuring that medications are efficiently and accurately dispensed and/or compounded and labeled.
  • Participate in clinical drug trials, including dispensing medications and completing associated documentation.
Requirements
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.
  • Broad knowledge of organization policies, procedures, and standards of care.
  • Knowledge of evidence-based clinical pharmacy practice, medication use evaluation, research processes, and methodology, and drug distribution processes in a hospital setting.
  • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision, communicate effectively both verbally and in writing, establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public, organize and prioritize work, operate related equipment, and perform other related duties as assigned.
