Job Title: Continuing Care Assistant

VON Canada is seeking a dedicated and compassionate Continuing Care Assistant to join our team.

Job Summary:

We are looking for a skilled and caring individual to provide high-quality care to our clients in their homes. As a Continuing Care Assistant, you will work closely with our clients, their families, and our healthcare team to deliver personalized care and support.

Responsibilities:
  • Provide personal care and support to clients, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation and household chores
  • Monitor and report any changes in client condition to the healthcare team
  • Collaborate with clients, families, and healthcare professionals to develop and implement care plans
  • Maintain accurate and confidential records of client care
  • Participate in ongoing education and training to enhance skills and knowledge
Requirements:
  • Continuing Care Assistant Certification
  • First Aid and CPR Certification
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to maintain confidentiality and handle sensitive information
  • Flexibility to work varied shifts, including evenings and weekends
