Patient Care Coordinator

We are seeking a compassionate Registered Practical Nurse (RPN) to join our healthcare team in West End Canada. In this role, you will provide quality nursing care to patients in various settings, including assessing patients' needs, developing individualized care plans, and administering medications and treatments as prescribed by physicians.
Key Responsibilities:- Assess patients' needs and develop care plans.
- Administer medications and treatments.

- Valid RPN license in Canada.
