Preventive Health Care Specialist

Vancouver Coastal Health is seeking a skilled Preventive Health Care Specialist to join our team at the North Shore Diabetes Education Centre. As a key member of our multidisciplinary team, you will play a vital role in promoting healthy lifestyles, preventing complications, and improving overall quality of life for our clients with chronic diseases.
The successful candidate will possess a strong understanding of nursing theory and practice, as well as comprehensive knowledge of evidence-based nursing practices related to diabetes prevention and management. You will work collaboratively with other healthcare professionals to develop, implement, and evaluate education programs aimed at promoting healthy lifestyles and self-management skills among our clients.

Responsibilities
- Develop and deliver individualized education plans, focusing on healthy lifestyle choices and self-management strategies.
- Conduct assessments and provide guidance on insulin pump therapy, as needed.
- Collaborate with interdisciplinary teams to ensure seamless client care and effective communication.
- Participate in quality improvement initiatives, contributing to the enhancement of our services and patient outcomes.
Requirements
- Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Minimum two years' experience in community or outpatient care settings, preferably with a focus on chronic disease management.
- Eligibility for accreditation as a Certified Diabetes Educator (CDE) is highly desirable.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes, if required.
