Primary Healthcare Professional

We are seeking a highly skilled and compassionate Primary Healthcare Professional to join our team at Health Careers in Sask. As a Nurse Practitioner, you will play a vital role in providing clinical support related to high-risk behaviors and chronic disease management for individuals with significant barriers to accessing healthcare. Your expertise will also involve working with community partners to improve the health of Saskatoon's core neighborhoods.
Key Responsibilities:
- Provide clinical support related to high-risk behaviors and chronic disease management
- Collaborate with community partners to improve health outcomes
- Support referrals to other health services as needed
Requirements:
- Two years of advanced RN (NP) experience in a relevant area
- Three years of full-time experience as a Registered Nurse
- Valid Class 5 driver's license
- Basic Life Support Certificate (BLS)
Preferred Qualifications:
- One year of nursing experience in a primary healthcare setting
