Job Title: Counsellor - Dual Diagnosis

Job Summary:

Pathways to Independence is seeking a highly skilled Counsellor - Dual Diagnosis to provide case management and assisted living support for adults with a dual diagnosis, mental health, and behavior challenges. The successful candidate will work under the direction of the Client Services Manager to plan and implement services and supports to enhance a person's ability to choose and promote self-determination and independence.

Key Responsibilities:

  • Case Management: Develop and implement individualized plans to support adults with a dual diagnosis, mental health, and behavior challenges.
  • Assisted Living Support: Provide daily living support to adults with a dual diagnosis, mental health, and behavior challenges, including assistance with personal care, medication management, and daily activities.
  • Community Living Support: Support adults with a dual diagnosis, mental health, and behavior challenges to live independently in the community, including assistance with daily living tasks, transportation, and accessing community resources.
  • Collaboration and Communication: Work collaboratively with the support team, including family members, caregivers, and other professionals, to ensure that every supported person understands their rights and responsibilities and receives services and supports based on their needs, interests, and goals.
  • Documentation and Reporting: Maintain accurate and up-to-date records of client information, including progress notes, treatment plans, and other relevant documentation.

Requirements:

  • Education: A post-secondary diploma or degree from a human services field and specialized education, training, or certification in developmental disabilities, disability studies, behavior management, social work, or psychology.
  • Experience: Experience working with adults with a dual diagnosis, mental health, or other cognitive impairment in a residential, community, or independent living environment.
  • Skills: Strong communication and interpersonal skills, with the ability to work effectively with adults with a dual diagnosis, mental health, and behavior challenges, as well as their families and caregivers.
  • Certifications: Current First Aid and CPR certification, valid G class driver's license, and proof of full COVID vaccination.
