About the Acute Care Medicine Unit for Older Adults

The Acute Care Medicine Unit for Older Adults is an expanded service of Geriatric Medicine on 9C/D. Our care model is designed to meet the growing needs of care for the older adult population with acute illnesses.

Job Summary

We are seeking a skilled Nurse Practitioner to join our multidisciplinary team. As a key member of our team, you will play a vital role in assisting older adults with their acute healthcare needs, including providing care to those who have experienced significant functional decline in a short time frame, or who are experiencing multiple complex medical and/or psychiatric problems.

Key Responsibilities
  • Diagnose and treat previously undiagnosed patients for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice.
  • Monitor ongoing care, order appropriate screening diagnostic investigations, and interpret reports of investigations.
  • Establish priorities for management of health, diseases, disorders, and conditions, and provide follow-up treatment.
  • Communicate with patients and families about health findings, diagnoses, and priorities, outcomes, and prognoses.
  • Collaborate and consult with medical staff colleagues, the healthcare team, and social service providers to determine the appropriate resources necessary for patient status.
  • Develop and implement treatment plans, and may admit and discharge patients to facilities according to medical staff bylaws.
  • Prescribe drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable PHC medical staff rules/credentialing/privileging.
  • Provide mentorship and informal education related to patient care to registered nurses.
  • Participate in research and quality improvement contributing to patient care, access, and flow, as well as organizational quality and medical staff quality initiatives.
  • Maintain population health focus by implementing screening and health promotion activities for populations at risk, patients, and families served.
  • Participate in the education of nurse practitioner students and other healthcare professionals through preceptorship, role modeling, and knowledge development and exchange in clinical practice, in classroom environments, and community environments to foster healthcare partnerships.
  • Develop, implement, and evaluate policies and procedures related to medical staff quality, organizational development, advanced practice nursing, nursing, interdisciplinary care, and health system practices.
  • Complete Encounter Records for the purpose of evaluating Nurse Practitioner implementation as a requirement of the Ministry Of Health.
Requirements
  • Current registration as a Nurse Practitioner with the British Columbia College of Nursing Professionals.
  • Successful completion or awaiting results of the Objective Structured Clinical Examination (OSCE).
  • Completion of the Basic Cardiac Life Support Course, Level C.
