Job Title: Adult Day Program Worker

Job Summary:

The Adult Day Program Worker is a vital member of the healthcare team at Prairie Mountain Health, working under the direction of the Care Team Manager to deliver exceptional service and quality care to our clients. This role requires a strong commitment to service excellence and continuous quality improvement.

Key Responsibilities:

  • Develop, coordinate, and implement activities to meet the social, recreational, and therapeutic needs of participants.
  • Collaborate with the Recreation Lead and Home Care Case Coordinator to assess participant needs and develop programs.
  • Provide a monthly program calendar to participants and ensure a safe and comfortable environment for all programs and activities.
  • Coordinate transportation and food services as needed, and maintain accurate records and statistics.
  • Participate in in-services, workshops, and meetings to stay up-to-date on Adult Day Program best practices.
  • Support an environment that promotes patient safety and well-being, and demonstrates a commitment to caring, health, and wellness.

  • Ability to work collaboratively as part of a multidisciplinary team.
  • Flexibility and adaptability in a fast-paced environment.
  • Basic knowledge of healthcare principles and practices.
