Island Health is seeking a part-time Family Practitioner to deliver outreach services in Sooke, B.C. The successful candidate will provide accessible primary episodic care services to adults and children impacted by poverty, colonization, stigma, and homelessness.
Key Responsibilities:
- Deliver outreach services to a broad range of clients, including adults and children
- Provide primary care services, including OAT and medical care
- Work collaboratively with a primary care RN, an MHSU RN, and a MOA to support clients
- Actively work towards decreasing barriers and social determinants of health in accessing care
Requirements:
- Family Practitioner certification
- Experience working with vulnerable populations
- Strong communication and interpersonal skills
