Job Summary:

We are looking for a highly motivated and experienced Nurse Practitioner to provide primary collaborative healthcare to our residents and their families. The successful candidate will work collaboratively with our interdisciplinary team to assess, diagnose, and provide intervention for diseases, disorders, and conditions.

Key Responsibilities:
  • Provide primary collaborative healthcare to an agreed-upon group of residents and their families
  • Communicate findings and diagnoses to residents, families, and the interprofessional care team
  • Assume a leadership role within the program and the interdisciplinary team to develop solutions and service delivery models
  • Advocate for residents at the point of care, across the organization, and at the health system/policy level
  • Encourage and model critical clinical inquiry to drive the development of nursing research and improve resident care
Qualifications:
  • Nurse Practitioner-Primary Health Care ('NP-PHC') or Nurse Practitioner-Adult ('NP-Adult') certificate from a recognized program
  • National certification in Gerontology Nursing from the CAN is preferred
  • Minimum two (2) years providing geriatric care in one of the following settings: long-term care, community, homecare, and virtual care
  • Experience working in long-term care is considered an asset
  • Demonstrated competency in autonomous diagnosis, order, and interpretation of diagnostic tests, prescribing pharmaceuticals, and performing procedures within the RN (EC) legislated scope of practice, designated medical directives, and Collaborative Practice Agreement
  • Advanced competence with computers and mobile communications is required
  • Demonstrated effective leadership skills, including flexibility, creativity, and commitment
  • Excellent interpersonal skills, including dealing with conflict, mentoring, and coaching
  • Demonstrated research knowledge, program planning, implementation, and evaluation skills
