About the Role
Vancouver Coastal Health is seeking a skilled Nurse Specialist to join our team at Hilltop House Long Term Care Facility in Squamish, BC.
Key Responsibilities
- Provide high-quality nursing care to residents in a Long Term Care setting, utilizing sound judgment and critical thinking skills.
- Develop and implement care plans in collaboration with the interdisciplinary team, ensuring resident-centered care and promoting quality of life.
- Act as a resource for the team, providing information and coordination of ongoing resident care, and supporting the work of RCA's and LPN's.
- Enhance the quality of life of residents and their families through effective communication, counseling, and teaching.
- Consult, confer, and collaborate with other healthcare providers to ensure comprehensive care.
- Maintain and advance clinical competence through ongoing education and professional development.
- Participate in designated committees and research projects as assigned.
Requirements
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of an approved gerontology course or an equivalent combination of education, training, and experience.
- Demonstrated ability in nursing practice related to residents with chronic diseases.
- Demonstrated knowledge of pathophysiology, aging changes, polypharmacy, treatment protocols, and nursing interventions related to common diagnoses and specific care situations.
- Demonstrated ability to provide nursing assessments and interventions related to common diagnoses and specific care situations.
- Demonstrated ability in recognizing physical adaptations required to accommodate the frail elderly and/or physically impaired resident population.
- Demonstrated ability in End of Life care approaches, Embedding Palliative Approach in Care (EPAC) in long term care and systems of family dynamics and end-of-life issues.
- Demonstrated ability in Chronic Disease Management, anticipating and responding to potential complications and care issues.
- Demonstrated ability in correct usage of equipment such as lifts, oxygen, suction machines, pumps, and other related equipment.
- Demonstrated ability in developing care plans and implementing care in relation to resident and family priorities.
- Demonstrated skill in the assessment and management of residents with acute/chronic pain management, including residents with cognitive impairment.
- Demonstrated skill in safe care and management of residents with Oxygen Therapy, VAC Dressing, Wound Care, Catheters, Ostomy Care, Tracheostomy Care, Ventilator Management, and other treatments.
- Demonstrated skill in performing RN autonomous scope of practice and standards of care relevant to designated patient/client population.
