The Role:

We are seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team. As a Physiotherapist - Community Care Specialist, you will play a critical role in providing high-quality patient care and support in the community.

Key Responsibilities:

  • Conduct professional assessments of patients in their homes and communities
  • Develop and maintain effective care plans in collaboration with the care team
  • Work with patients to set and achieve time-specific, measurable goals and outcomes
  • Provide one-on-one therapeutic support and promote independence in the home and community
  • Collaborate with the care team to ensure seamless care coordination

Requirements:

  • Master's Degree in Physiotherapy from a recognized university
  • Registration with the College of Physiotherapists of Ontario
  • Proven computer skills and ability to work with electronic health records
  • Strong assessment and treatment skills, with ability to work independently and as part of a team
  • Excellent English verbal and written communication skills
  • French language skills are an asset
