Job Title: Nurse Practitioner

Baycrest is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a Nurse Practitioner, you will play a vital role in providing high-quality patient care and contributing to the development of our geriatric care programs.

Responsibilities:
  • Provide primary collaborative healthcare and follow-up to an agreed-upon group of residents and their families within the Long-Term Care (LTC) home.
  • In collaboration with attending physicians, communicate findings and diagnoses and discuss health outcomes and prognoses to residents, families, and the interprofessional care team.
  • Assume a leadership role within the program and the interdisciplinary team to develop solutions/service delivery models that provide exceptional resident/family care.
  • Provision of leadership in advocating for residents at the point of care, across the organizations, and/or at the health system/policy level.
  • Encourage and model critical clinical inquiry that drives the development of nursing research and improves resident care.
  • Identify needs and make recommendations for changes within the system to enhance both clinical practice and resident outcomes.
Qualifications:
  • Nurse Practitioner-Primary Health Care ('NP-PHC') or Nurse Practitioner-Adult ('NP-Adult') certificate from a recognized program.
  • National certification in Gerontology Nursing from the CAN is preferred.
  • Minimum two (2) years providing geriatric care in one of the following settings: long-term care, community, homecare, and virtual care.
  • Experience working in long-term care is considered an asset.
  • Demonstrated competency in autonomous diagnosis, order, and interpretation of diagnostic tests, prescribing pharmaceuticals, and performing procedures within the RN (EC) legislated scope of practice, designated medical directives, and Collaborative Practice Agreement.
  • Advanced competence with computers and mobile communications is required.
  • Demonstrated effective leadership skills – including flexibility, creativity, and commitment.
  • Excellent interpersonal skills, including dealing with conflict, mentoring, and coaching.
  • Demonstrated research knowledge, program planning, implementation, and evaluation skills.
