Licensed Practical Nurse

Vancouver Coastal Health is seeking a Licensed Practical Nurse (LPN) to join the Downtown Community Health Centre, Integrated Care Team serving the Downtown Eastside Community in Vancouver, BC.
Key Responsibilities- Work collaboratively across multiple sites, clinics, and services, or on outreach/home based visits within an integrated health service delivery model and interdisciplinary care team, to provide holistic care with primary care clients.
- Care for clients and families who are experiencing health challenges across the primary care continuum.
- Seek direction/guidance from a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Nurse Practitioner (NP), or Physician (MD) in situations where client care needs are not stable or predictable.
- Work closely with the RN/RPN/NP/MD in direct care activities using a team-based care approach, such as assessing, planning, implementing, evaluating and documenting client care as part of the team and collaboratively with other Community Care Services.
- Establish, maintain and enhance therapeutic relationships based on respect.
- Support clients and their families to achieve and maintain optimal health.
- Graduation from a recognized program for Practical Nurses. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year of recent related experience or completion of a full final clinical placement in a community setting, working as a Licensed Practical Nurse providing care through a holistic mental health, substance use and/or primary care lens, or an equivalent combination of education, training and experience.
- Valid BC Driver's License preferred.
