Wound Ostomy Continence Nurse

As a Registered Nurse in Wound, Ostomy and Continence at SE Health, you will be part of a dynamic team that provides specialized care to individuals with ostomies, acute and chronic wounds, and urinary and fecal continence concerns.
Key Responsibilities- Provide holistic assessment and management of clients with wound, ostomy, and continence needs
- Develop and implement individualized plans of care in collaboration with the interprofessional team
- Provide education and mentorship to clients, families, and healthcare providers
- Collaborate with the Wound Ostomy Resource Nurse (WORN) and K2P Wound Care Resource Nurses (K2P WCRN) to optimize client outcomes
- Participate in quality improvement initiatives to enhance the delivery of wound, ostomy, and continence care
- Current registration with the College of Nurses of Ontario
- Completion of the Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program
- Ability to work collaboratively as part of an interprofessional team
- Strong communication and interpersonal skills
