Position Summary
As a Clinical Nutrition Specialist, you will play a vital role in providing comprehensive nutritional care and guidance to patients, residents, and clients within our healthcare setting. You'll collaborate closely with multidisciplinary teams to develop personalized nutrition plans, educate individuals on healthy eating habits, and contribute to the overall well-being of those under our care.
Responsibilities
- Patient Care: Conduct thorough nutrition assessments, create individualized nutrition care plans, monitor progress, and provide ongoing education to patients, residents, and clients.
- Team Collaboration: Actively participate in multidisciplinary team meetings, including care conferences, clinical rounds, and discharge planning, to ensure coordinated and holistic patient care.
- Program Development & Improvement: Contribute to the development and implementation of nutrition-related programs, policies, procedures, menus, and educational materials.
- Clinical Expertise: Provide expert nutritional guidance and support to healthcare professionals and contribute to the advancement of best practices in clinical nutrition.
