Job Summary

We are seeking a highly skilled and compassionate Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health. As a Community Mental Health Specialist, you will play a vital role in providing treatment and care management services to individuals with mental illness and concurrent disorders.

Key Responsibilities
  • Coordinate care for clients by collaborating with interprofessional care teams, performing intake functions, and conducting mental health assessments.
  • Develop, implement, and maintain comprehensive treatment plans based on client needs and best practices.
  • Administer prescribed medications and monitor and assess clients' mental state, response to medications, and efficacy of treatment plans.
  • Provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and developing care plans.
  • Work collaboratively with clients and professionals to develop and implement individualized care plans and assist clients to access appropriate services in the community.
  • Establish, maintain, and enhance therapeutic relationships with clients and their informal support networks.
  • Teach clients and their families about management and treatment of specific psychiatric conditions, concurrent substance use issues, and their symptoms.
  • Provide treatment services, including crisis intervention, individual counseling, and education, according to various theoretical models.
  • Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team.
  • Act as a client advocate to protect and promote clients' rights to autonomy, respect, privacy, dignity, and access to information.
Qualifications
  • Graduation from an approved School of Nursing or Psychiatric Nursing.
  • Two years of recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Valid Class 5 BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
Skills and Abilities
  • Ability to complete initial and ongoing mental health and substance use client assessments and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions.
  • Knowledge of mental illness and substance use treatment, psychopharmacology, counseling, and group facilitation skills.
  • Ability to communicate effectively verbally and in writing with clients, families/caregivers, the public, medical staff, and members of the interprofessional care team.
  • Ability to promote person-centered care that demonstrates empathy for clients/families/caregivers, sensitivity to diverse cultures and preferences, including advocacy for special populations.
  • Ability to educate clients and families/caregivers about topics essential to their health care and well-being.
  • Ability to organize and prioritize work, collaborate with and promote cooperation among members of the interprofessional care team, and use factual information and basic principles and procedures to support nursing decisions and actions.
