Position Overview
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team in the North Simcoe Sub-Region. As a key member of our healthcare network, you will play a vital role in delivering holistic patient care within the community setting.
Responsibilities
- Provide comprehensive primary care services to patients with diverse medical needs, including palliative care for those nearing end of life.
- Conduct thorough clinical assessments, monitor patient progress, and manage acute and chronic conditions effectively.
- Collaborate closely with patients, families, and other healthcare professionals to develop individualized care plans.
- Advocate for patient well-being and ensure access to appropriate resources and support services.
- Maintain accurate and detailed documentation of all patient interactions and care provided.
Qualifications
- Active membership in good standing with the College of Nurses of Ontario.
- Registered Nurse Extended Class (RN EC) designation.
- Demonstrated knowledge of community health resources and healthcare delivery systems.
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
- Strong organizational and time management abilities, with the ability to work independently and as part of a team.
- Proficiency in a Windows environment.
Preferred Qualifications
- Specialized education in palliative pain and symptom management.
- Experience in palliative/end-of-life care.
- Familiarity with Medical Assistance in Dying legislation.
- Advanced assessment skills and knowledge of clinical therapeutics.
- Demonstrated clinical leadership and collaborative practice with all healthcare providers.
