Job Summary:
The Transitional Care Nurse will be responsible for providing high-quality nursing care to clients in transitional care settings. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Key Responsibilities:
- Develop and implement admission and discharge protocols to ensure seamless transitions into and out of service locations.
- Act as a liaison with community partners, hospitals, and next-level care providers to ensure continuity of care.
- Develop Service Agreements and Transitional Plans with clients, primary care givers, and healthcare partners in accordance with regulations and best practice approaches in transitional care.
- Facilitate the accurate sharing of client-specific information between healthcare providers and care teams to ensure continuity of care, client safety, and well-being.
- Assist with the review of nursing standards and protocols, and provide in-service training and daily coaching to healthcare teams to ensure application of same.
- During times of insufficient staffing, may be required to provide direct nursing care.
Requirements:
- Completion of a vocational, college, or other approved program for licensed practical nurses and registration with a regulatory body required.
- Must have successfully completed the Canadian Practical Nurses Registration Examination and possess a current Registered Practical Nursing Certificate of Competence for the Province of Ontario.
- Prior experience in geriatric, rehabilitation, or working with adults with disabilities an asset.
- Ability to work within legislation pertinent to the operations of a healthcare facility, including but not limited to such acts as Public Health, Mental Health, Resident's Protection, and other federal, provincial, and municipal statutes.
- Must demonstrate friendly, professional, and business-like demeanor and have a sound understanding and empathy for the needs of older adults and their primary care givers.
- Prior knowledge of using a variety of healthcare data management software desirable.
- Must be able to communicate effectively both verbally and in writing.
- Must demonstrate the ability to use tact and discretion and display professionalism at all times, demonstrate sound judgment and flexibility, and be able to solve problems effectively.
- Must be a strong team player with the ability to coach and inspire others.
- Must be able to meet physical requirements of the job, such as lifting and transferring of clients.
- A valid driver's license and reliable vehicle are requirements of this position.
- Will be required to provide a satisfactory Police Check, Vulnerable Screening Check, and Medical clearance certificate.
