About the Opportunity

We are seeking a skilled and compassionate Transfer of Care Clinician to join our Mental Health and Addictions team. As a key member of our team, you will play a vital role in facilitating the transfer of care for clients across various mental health and addiction services.

As a Transfer of Care Clinician, you will be responsible for coordinating all aspects of transitions of care, ensuring that clients are connected to the most appropriate services and programs to meet their needs. You will establish and maintain positive relationships with clients, families, transfer/referral sources, and designated services, responding to requests and concerns in a timely and compassionate manner.

You will also participate in quality initiatives in the program of care, engage in collaborative person-centered care, and utilize available information technology to support best practice. It is an expectation that you will act in a professional and ethical manner, following all recognized codes of ethics, standards, legislation, and guidelines relevant to clinical practice and profession.

About You

We are seeking a candidate who has a strong background in mental health and addiction services, with a minimum of 4 years of clinical experience. You will have a Bachelor's degree in a health-related discipline and be a member in good standing with a provincial self-regulated health college/governing body.

You will have excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals. You will also have a strong understanding of the Involuntary Psychiatric Treatment Act (IPTA) and be familiar with navigating the information technology systems of a large health organization.

Hours

This is a permanent full-time position, working 75 hours bi-weekly. You will be required to work a variety of shifts, including evenings and weekends, as part of a rotating schedule.

Compensation and Benefits

The successful candidate will be offered a competitive salary range of $33.97 - $42.89 per hour, depending on experience. You will also be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
