Health Services Integration Coordinator

About the Role:
Bigstone Health Commission is seeking a highly motivated and passionate individual to assume the role of Health Services Integration Coordinator. This position will conduct various administrative and program management tasks while working with our external government partners and be the primary access point in coordinating mental health delivery models for increased access and equitable delivery of services for Bigstone Cree Nation members and all individuals living in the region.
Key Responsibilities:
- Plan, coordinate, and facilitate special projects, programs, and activities that enhance community health needs.
- Liaise with clients to identify and define project requirements, scope, and objectives to ensure their health needs are being met.
- Work in collaboration with Alberta Health Services organizations, community, and other external agencies.
- Initiate, develop, and explore opportunities for greater collaboration, partnerships, and linkages with healthcare providers.
- Maintain work priorities and workplans that collaborate with the project or program to ensure all deliverables are being met.
- Commence a community consultation process with available mental health services.
- Obtain and analyze data from community feedback.
- Prepare proposals, reports, and project updates.
- Strong project management and development skills.
- Advanced computer skills in Microsoft Office Suite.
- Post-secondary education (Certificate or Diploma) related to Health Services field and/or equivalent.
- Minimum 2 years of experience in the Health Services field.
- Previous experience working for a First Nation community would be an asset.
What We Offer:
- Full-time, fixed-term contract.
- $17 per hour.
