Job SummaryWe are seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our Visiting Nurse Program in the North Area. As a Visiting Nurse, you will work independently to provide exceptional nursing services to patients in their homes, developing and maintaining effective care plans and communicating with patients, family members, and other healthcare professionals as needed.
Key Responsibilities- Develop and maintain effective care plans for patients in the community
- Provide exceptional nursing services to patients in their homes, including one-on-one care and virtual consultations
- Communicate and liaise with patients, family members, and other healthcare professionals as needed
- Perform other clinical duties as required
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
