The Home and Community Care Support Services Champlain is seeking a skilled Nurse Practitioner to join our team in providing high-quality primary care services to patients in need. As a key member of our Rapid Response Program, you will work collaboratively with multidisciplinary healthcare partners to deliver comprehensive care and support to at-risk patients.
Key Responsibilities- Deliver primary health care services, including medication reconciliations, prescribing medication, and ordering lab work, to patients in need.
- Liaise with existing patient physicians and community partners to address patient needs and goals.
- Provide health promotion and education to patients using a variety of methods tailored to their needs.
- Conduct focused assessments and provide recommendations to the Rapid Response Program and Home and Community Care Support Services Care Coordinators.
- Collaborate with the RRN Program leaders to develop and implement policies and procedures.
- Current registration with the College of Nurses of Ontario as a Registered Nurse (Extended Class) and eligibility to practice as a Nurse Practitioner.
- Related nursing experience in a home or community care environment, including linkages with community health agencies and other health disciplines.
- Experience with case management and discharge planning.
- Advanced assessment and diagnostic reasoning skills.
- Demonstrated advanced knowledge in consultation and ethical decision-making.
- Knowledge of Champlain HCCSS operations, quality, risk, and performance management principles.
- Knowledge of relevant healthcare legislation.
- Ability to accurately complete required documentation, reports, forms, and recommendations.
This is a hybrid model position with a combination of work locations, including community and clinical settings, patients' homes, on-site, and remote. The schedule is comprised of eight or nine hour shifts for a total of 35 hours bi-weekly, with one weekend per month.
