Job Title: Personal Support Worker - Long Term Care

We are seeking a highly motivated and compassionate Personal Support Worker to join our team at the City of Toronto. As a Personal Support Worker, you will play a vital role in providing high-quality care and support to our residents in a long-term care setting.

Key Responsibilities:
  • Provide personal care and support to residents, including bathing, dressing, grooming, and toileting.
  • Assist residents with daily activities, such as feeding, transferring, and ambulation.
  • Monitor and report on resident's emotional and physical condition, and communicate with the healthcare team as needed.
  • Participate in the development and implementation of care plans, and provide input on resident's needs and preferences.
  • Work collaboratively with the healthcare team to ensure resident's safety and well-being.
  • Provide emotional support and comfort to residents, and promote their emotional well-being.
  • Participate in social and recreational programs for residents, and assist with activities such as crafts, music, and exercise.
  • Maintain accurate records of resident's care and services provided.
Requirements:
  • Successful completion of a Personal Support Worker program or training that meets the vocational standards established by the Ministry of Training, Colleges and Universities.
  • Experience in providing personal care to ambulant and non-ambulant residents in a long-term or rehabilitation care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare staff.
  • Ability to work in a team environment and provide support to colleagues as needed.
  • Physical ability to lift, transfer, and move residents safely and efficiently.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
