Patient Care Coordinator

We are seeking a highly skilled and experienced Patient Care Coordinator to join our team at Fraser Health Authority. As a key member of our healthcare team, you will be responsible for providing clinical supervision, guidance, and support to nursing staff, other clinical team members, and support staff within the assigned unit(s).
Key Responsibilities- Apply established criteria and prioritize admissions/intake, lead and facilitate actions related to daily patient flow, plan for patient discharge, and ensure the care team follows through on specific care paths and processes.
- Supervise nursing and nursing support staff, coordinate the work of the clinical team, and monitor the quality of care and the safety of patients.
- Support quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education, and acting as a mentor and resource to staff.
- Initiate/lead/support plan of care conferences and rounds for the purpose of optimal patient care management according to best practice.
- Identify learning requirements of staff, provide input into the development of learning objectives, and provide clinical support and guidance.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training, and experience.
- Completion of post-graduate coursework and/or certification applicable to the designated clinical area.
