Health Care Technician - eHomecare Palliative/PediatricWe are seeking a dedicated and compassionate Health Care Technician (HCT) to join our dynamic eHomecare team. In this role, you will provide essential hands-on care to patients in their homes, under the guidance of a Registered Nurse. You will play a vital role in supporting individuals with diverse needs, including pediatric, palliative, complex care, and chronic disease management.
Key Responsibilities:- Provide personalized care and medication administration to patients in their homes.
- Collaborate closely with Registered Nurses, contributing to patient care plans and ensuring continuity of care.
- Meticulously document clinical data in electronic medical records, maintaining accurate and up-to-date patient information.
- Identify and coordinate additional resources needed within the home environment, working effectively with the office team.
- Foster positive relationships with patients and their families/caregivers, providing support and guidance throughout their care journey.
- Personal Support Worker (PSW) certification is required.
- 1-2 years of experience in palliative, pediatric, or acute care settings is preferred.
- Current CPR and First Aid certifications are essential.
- Demonstrated competency in a range of hands-on clinical skills.
- Ability to adapt to diverse home environments and patient needs.
- Excellent communication and problem-solving abilities.
- Experience with ongoing skills training and implementation.
- Access to a personal vehicle and a valid driver's license are required for travel between patient homes.
- Willingness to travel to neighboring communities is essential.
- A current criminal background check and appropriate Personal Protective Equipment (PPE) are mandatory.
