We are seeking a skilled Licensed Practical Nurse to join our Acute Care for the Elderly team at Peace Arch Hospital in White Rock, B.C. As a key member of our multidisciplinary team, you will provide high-quality patient care and work collaboratively with our experienced healthcare providers.
Responsibilities- Conduct comprehensive assessments of patients' physiological, psychological, socio-cultural, and spiritual needs.
- Develop and implement individualized care plans, collaborating with patients, families, and healthcare teams.
- Provide nursing care according to established policies, procedures, and standards, evaluating outcomes and making adjustments as needed.
- Participate in multidisciplinary care planning meetings and family conferences to ensure comprehensive care.
- Perform admissions, discharges, and transfers, maintaining accurate records and ensuring patient safety.
- Operate and maintain equipment related to patient care, ensuring a safe and clean environment.
- Assist in the orientation of new staff, students, and healthcare team members.
- Contribute to continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings, and committees.
- Graduate of a recognized practical nursing program or equivalent combination of education, training, and experience.
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Ability to communicate effectively, both verbally and in writing.
- Ability to deal with others effectively.
- Ability to organize work and implement a plan of care.
- Knowledge and competencies to work at full scope of practice as currently required by the BCCNM, including administering medications.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.
