About the Role

VHA Home HealthCare is seeking a highly skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist, you will play a critical role in providing consultation, assessment, and planned interventions for diverse client groups across Southern Ontario and the Capital region.

Key Responsibilities
  • Assess and diagnose clients in all age groups through the selection and administration of standardized and non-standardized tests.
  • Develop and implement individualized goal-directed treatment programs based on case history, assessment results, current professional practices, and College standards.
  • Monitor client progress and evaluate treatment and management on an ongoing basis.
  • Counsel and educate clients, caregivers, and family members regarding the nature and management of communication deficits.
  • Identify and teach self-management strategies to help clients recognize and avoid unsafe situations.
  • Discuss clinically significant information with physicians or other appropriate members of the healthcare team.
  • Assist in client discharge planning in conjunction with the client, family members, and other members of the healthcare team.
  • Maintain accurate and comprehensive written documentation of client care in accordance with existing legal, professional, and VHA standards.
  • Maintain knowledge of current developments in Speech Language Pathology and Community Services appropriate for treatment populations.
  • Adhere to the code of ethics of the profession, maintaining standards of practice for the profession as defined by the Ontario College of Social Workers and Social Service Workers.
Requirements
  • Master's Degree or equivalent in Speech Language Pathology.
  • Membership in good standing with the College of Audiologists and Speech Language Pathologists of Ontario.
  • Eligible for membership in the Ontario Association of Speech Language Pathologists and Audiologists.
  • Minimum of two years clinical experience and knowledge of community care requirements and resources available.
  • Expertise in the management of clients with complex communication disorders.
  • Clinical expertise to provide a variety of speech language pathology interventions.
