CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse in Community Health Services. As a Visiting Nurse, you will have the opportunity to provide exceptional nursing services to patients in their homes, clinics, retirement homes, and schools.
Key Responsibilities- Develop and maintain effective Care Plans with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).
- Provide exceptional nursing services to patients, both in-person and virtually.
- Communicate and liaise with patients, family members, and others within the interdisciplinary care team.
- Perform other clinical duties as required.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- A strong desire and commitment to making a difference in the lives of our patients.
- Exceptional interpersonal skills.
- A valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record, including a 2-step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
