Role Summary

The Personal Support Worker (PSW) delivers essential personal assistance and homemaking services to clients and their families, adhering to the policies, procedures, and standards set forth by the organization. The PSW works collaboratively with healthcare professionals to ensure clients achieve a high level of independence and well-being.

This position qualifies for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent in direct client care.

Key Responsibilities
  1. Assess and review the client's or family's living situation and care plan to identify necessary adjustments and ensure their needs are met, reporting findings to the supervisor.
  2. Assist in creating the personal support care plan under the guidance of the Personal Support Program Manager, utilizing information from the funder, Manager, and the client or family.
  3. Implement modifications to the care plan as directed by the Manager, ensuring safety and compliance.
  4. Communicate any changes in the client's or family's support needs or living conditions to the supervisor or Program Manager.
  5. Perform personal support tasks tailored to the client and family’s needs, which may include light housekeeping, meal preparation, laundry, shopping, and ensuring household safety.
  6. Provide personal care as instructed by the Program Manager or supervisor, assisting clients with mobility, hygiene, and daily living activities.
  7. Administer basic bedside care under the supervision of a nurse when necessary.
  8. Offer respite care to clients as needed.
  9. Support clients' families by providing childcare as required for family relief.
  10. Organize daily activities, prioritizing personal support tasks under the direction of the Program Manager or supervisor.
  11. Complete daily check-ins and necessary documentation to ensure effective operation of personal and home support services.
  12. Evaluate the client's communication patterns and ability to engage.
  13. Coordinate with healthcare team members regarding the management of the client's homemaking, physical, and emotional needs, informing the supervisor or Manager of any changes or concerns.
  14. Adhere to Occupational Health and Safety regulations, as well as organizational policies and procedures.
  15. Follow risk management policies and procedures designed to promote client safety.
  16. Perform additional duties as assigned.
Relationships
  1. Regularly engage with the immediate supervisor or Personal Support Program Manager regarding necessary changes to client care plans and other pertinent information.
Qualifications and Experience:
  1. High school diploma.
  2. Preferred secondary school education.
  3. CCA Certification required in Nova Scotia.
  4. Certification in Home Support Worker (Levels I, II, III), Personal Support Worker program, or Health Care Aide is mandatory.
  5. Current Standard First Aid Certificate or CPR level C Certificate, as applicable.
  6. Experience in household management and working with elderly, ill individuals, or children.

  1. Excellent verbal and written communication skills.
  2. Ability to communicate effectively with clients and families, demonstrating respect and empathy.
  3. Strong assessment capabilities.
  4. Knowledge of community resources.
  5. Ability to work well in a multidisciplinary team or independently as needed.
  6. Willingness to accept direction.
  7. Adaptability to various social and economic environments.
  8. Commitment to maintaining confidentiality.
Additional Requirements:
  1. Access to a personal vehicle for work, along with a valid driver's license and proof of insurance.
  2. Current and original copy of a satisfactory Criminal Records Check.
  3. A Vulnerable Sector Search and/or Child Abuse Registry Check may be necessary.
  4. Availability for flexible working hours.
  5. Ability to wear Personal Protective Equipment (PPE) as required.
Physical Demands:
This position requires a detail-oriented approach in a dynamic environment, involving physical activities such as lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role will need to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine motor skills.
