Job Summary

As a key member of the Vitalité Health Network team, the Infirmier Clinicien en Diabète will play a vital role in promoting the health and well-being of patients with diabetes. This dynamic individual will be responsible for initiating the nursing process and managing patient care episodes within a supportive relationship framework, with the goal of empowering patients to take control of their diabetes management and preventing related complications.

Key Responsibilities
  • Collaborate with the Regional Chronic Disease Program Manager to develop and implement nursing care plans that meet the unique needs of patients with diabetes.
  • Provide high-quality clinical care that adheres to established nursing standards, philosophies, and objectives, with a focus on promoting patient autonomy and self-management.
  • Act as a role model, educator, and resource person for patients, primary care providers, direct care providers, and community groups, promoting best practices in diabetes management, including insulin pump therapy.
  • Develop and deliver specialized diabetes education programs based on best practices and evidence-based guidelines from the Canadian Diabetes Association, with a focus on nutrition and patient empowerment.
  • Conduct comprehensive assessments of patients with diabetes, identifying their unique needs and developing individualized care plans that address their physical, emotional, and social needs.
  • Collaborate with interdisciplinary teams to develop and implement care plans that meet the complex needs of patients with chronic diseases, including diabetes.
  • Provide education and support to patients and their families on diabetes self-management, including insulin therapy, blood glucose monitoring, and healthy eating habits.
  • Stay up-to-date with the latest research and evidence-based practices in diabetes care, incorporating this knowledge into practice to ensure high-quality patient care.
  • Participate in quality improvement initiatives to enhance patient outcomes and promote best practices in diabetes care.
Requirements
  • Current membership in the New Brunswick Nurses Association.
  • Valid certification in SIR (Système d'Information des Ressources).
  • Bachelor's degree in Nursing.
  • Minimum of five years of clinical experience working with patients with complex chronic diseases, including two years of experience working with patients with diabetes.
  • Successful completion of certification programs for delegated medical acts and advanced nursing skills, such as insulin pump therapy.
  • Certification in diabetes education, as per the established timeline.
  • Certification in the New Brunswick Insulin Adjustment Program, as per the established timeline.
  • Excellent communication and teaching skills.
  • Strong time management and prioritization skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and good driving record.
  • Ability to work in a fast-paced environment and adapt to changing priorities.
  • Good work history, including a strong attendance record.
  • Adherence to the principles of professional ethics, the management philosophy of Vitalité Health Network, and the organization's values.
  • Respect for the confidentiality policies of Vitalité Health Network.
