Oncology Clinical Pharmacist
The Provincial Health Services Authority (PHSA) is seeking a dedicated Oncology Clinical Pharmacist to join our specialized oncology team. This role is pivotal in enhancing patient care through tailored medication management and collaboration with healthcare professionals.
Key Responsibilities
- Customize drug therapy for patients by reviewing medical records, evaluating clinical outcomes, and working closely with healthcare teams to formulate individualized therapeutic strategies.
- Engage with patients and healthcare providers to ensure optimal medication therapy, which includes developing discharge plans for hospitalized patients and medication schedules for outpatients, as well as addressing patient inquiries regarding treatment plans.
- Accurately prepare and dispense medications, ensuring compliance with relevant health regulations and standards.
- Provide comprehensive drug information to healthcare professionals and patients, including personalized counseling to support medication understanding and adherence.
- Collaborate with the care team to facilitate access to non-BCCA benefit medications, offering guidance on financial support options and compassionate use programs.
- Mentor pharmacy students and residents, fostering their professional development through supervision and educational support.
Qualifications
- A Bachelor's Degree in Pharmacy with active registration as a Licensed Pharmacist in British Columbia.
- Completion of a Hospital Pharmacy Residency Program.
- A minimum of one year of relevant experience or an equivalent combination of education and training.
Additional Skills
- Proficiency in aseptic techniques and safe handling of hazardous medications.
- Strong organizational skills and the ability to work autonomously.
- A commitment to fostering reconciliation and understanding of Indigenous health issues, along with knowledge of relevant cultural safety practices.
