Clinical Social Worker - Long Term Care Team

Position Overview:
We are looking for a passionate Clinical Social Worker to become part of our Long Term Care Team. In this role, you will engage with clients and their families to formulate and execute comprehensive care strategies, deliver direct support, and assist in navigating available community resources. Your contribution will be vital in fostering healthier lives within our community.
Key Responsibilities:
- Work collaboratively with clients and their families to set achievable care objectives
- Evaluate eligibility for services and create tailored care plans
- Apply social work methodologies and evidence-based approaches
- Offer counseling and emotional support to clients and their families
- Coordinate efforts with healthcare professionals and community service providers
Required Qualifications:
- Master's degree in Social Work from an accredited institution
- Active registration with the British Columbia College of Social Workers
- Experience in clinical practice within Home Health
- Valid BC Driver's License
- Understanding of adult health demographics and community resources
- Proficient in working autonomously as well as part of a collaborative team
