About the Role

Community Therapists is seeking a skilled Occupational Therapist to join our team in a community-based rehabilitation role. As an Occupational Therapist, you will have the opportunity to work with clients across the rehabilitation continuum, providing a diverse range of services to support their independence and well-being.

Key Responsibilities

  • Conduct hospital discharge planning and assessments to ensure seamless transitions to community-based care
  • Develop and implement cognitive assessments and rehabilitation plans to increase client independence
  • Create graduated activity plans to enhance home and community functioning
  • Design and implement community integration plans to support clients in achieving their goals
  • Provide case management services to facilitate access to other health services
  • Develop and supervise rehabilitation assistant plans to support client care
  • Liaise with funders, family, and healthcare teams to ensure comprehensive care

Requirements

  • Member of the College of Occupational Therapists of British Columbia (COTBC) or eligible for registration
  • Valid driver's license and access to a reliable vehicle
  • Excellent verbal and written communication skills
  • Aptitude for negotiation and consensus building
  • Experience working with insurance fee payers is an asset
  • Experience working with clients with traumatic brain injury (TBI) is an asset
