We are seeking a highly skilled and experienced Social Worker to join our team at Fraser Health. As a Sexual Health and Blood Borne Infections Social Worker, you will play a critical role in providing clinical and consultative case management services to individuals living with HIV and/or HCV.
Key Responsibilities- Provide advanced psychosocial assessments, counseling, and referrals to clients and families living with HIV.
- Develop and implement individualized care plans that address the client's needs and promote overall health stabilization and physical and emotional well-being.
- Collaborate with the interdisciplinary team to provide comprehensive care and support to clients and families.
- Provide education and consultation to healthcare professionals and community agencies on psychosocial factors affecting client care engagement.
- Develop and maintain effective relationships with community social service agencies to facilitate communication and decision-making around client care.
- Participate in program evaluation and quality improvement initiatives to enhance system processes and resources for HIV-positive individuals.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two years recent, related experience in a complex healthcare environment, including one year experience working with marginalized clients with multiple health issues, including HIV, mental health, and substance use issues.
- Valid B.C. driver's license and access to a personal vehicle for business-related purposes.
- Comprehensive skills in trauma-informed practice and intervention.
- Knowledge and understanding of emotional and mental health issues related to illness, injury, and client and family cultural dynamics.
- Comprehensive knowledge of other health disciplines and their role in HIV care, prevention, and in a population and public health framework.
- Ability to educate and provide consultation to clients, families, and healthcare professionals.
- Ability to develop, coordinate, and deliver in-services, presentations, psycho-education, and support to groups.
