Vancouver Coastal Health is seeking dedicated Registered Nurses to become part of the Home Health team. This role is pivotal in delivering comprehensive care to clients in the community.

Key Responsibilities:

  • Provide care for individuals with acute, chronic, and end-of-life health needs.
  • Collaborate with clients and their families to set achievable and safe health goals through thorough assessment, planning, and coordination of nursing services.
  • Evaluate service eligibility and assist in creating, implementing, and modifying care plans tailored to individual needs.
  • Deliver direct nursing care and facilitate care management to empower clients in managing their health and accessing community resources.
  • Engage with other healthcare partners and apply current evidence-based practices in your nursing approach.

Qualifications:

  • Active registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • A minimum of one year of recent clinical experience in a relevant field, particularly with complex health conditions, or an equivalent combination of education and experience.
  • A valid BC Driver's License is essential for local travel as required.

Skills and Competencies:

  • In-depth knowledge of community health nursing principles and practices within a client-centered care framework.
  • Ability to conduct client assessments and interventions with a focus on trauma-informed and culturally safe care.
  • Strong critical thinking skills to assess safety and risk in various environments.

  • Capability to prioritize and manage multiple clients while fostering cooperation and innovation.

  • Proven ability to work collaboratively within an interdisciplinary team to address diverse health challenges.
  • Proficient in documenting client information accurately in electronic medical records.
  • Commitment to maintaining confidentiality and ethical standards in all client interactions.
  • Strong communication skills to build trust and rapport with clients, families, and healthcare professionals.
  • Experience in teaching and coaching clients and families in self-management of care.
  • Familiarity with provincial regulations and community resources related to health services.
  • Knowledge of health management strategies for various conditions, including chronic diseases and mental health.
  • Demonstrated organizational skills to effectively plan and prioritize work tasks.
  • Competence in CPR and relevant nursing techniques.
  • Physical capability to fulfill the responsibilities of the position.
