Responsibilities:- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to ensure proper follow-through and documentation of resident care services.
- Assist in conducting resident evaluations and gathering data to inform care decisions.
- Monitor and respond to resident incidents, documenting and reporting as required.
- Communicate effectively with residents, families, and healthcare providers to ensure medical needs are met.
- Maintain accurate and up-to-date resident wellness files, adhering to Sunrise policies and regulations.
- Provide clinical support and assistance to community team members as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum one year of experience in a healthcare setting, preferably in home health, assisted living, hospital, or long-term care.
- Strong knowledge of nursing practices, techniques, and methods applied to resident care.
- Excellent communication and interpersonal skills.
- Ability to work weekends, evenings, and flexible hours to meet resident needs.
