Job Summary
We are seeking a skilled Registered Practical Nurse to join our team at St. Joseph's Care Group. As a Geriatric Assessment and Rehabilitation Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our team.
Key Responsibilities:
- Provide comprehensive geriatric assessments and develop individualized care plans to meet the unique needs of our patients.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and excellent patient outcomes.
- Conduct physical assessments and administer medications as required.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure optimal care and support.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Current Certificate of Registration with the Ontario College of Nurses.
- Current CPR certification.
- Current physical assessment and pharmacology courses.
- Clinical knowledge and demonstrated ability to work with designated populations.
- Excellent interprofessional team skills and ability to work collaboratively.
- Must work within the full scope of practice as an RPN.
- Background in Comprehensive Geriatric Assessments and experience working with seniors considered an asset.
- Demonstrated regular attendance at work.
Condition of Employment:
- This position is conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- Offers of employment made to external candidates are conditional upon providing proof of COVID-19 vaccination as per the or proof of valid medical exemption, prior to the offered effective date. If proof of vaccination or valid medical exemption is not provided, the offer of employment is rescinded.
