The Patient Care Facilitator plays a crucial role in building and maintaining a positive connection between the clinic and its clientele, which includes patients, staff, funding bodies, and referral sources. This is accomplished by delivering efficient administrative assistance and coordinating clinic operations with a warm, enthusiastic, and professional demeanor towards all patients, team members, and external stakeholders.
The Patient Care Facilitator is accountable for managing all facets of patient onboarding, data management, responding to inquiries, making outbound calls, handling incoming faxes and emails, as well as processing invoices and payments.
Key Responsibilities
Respond to phone inquiries, welcome patients and visitors upon arrival, and ensure outstanding customer service for all inquiries or issues Oversee a thorough and precise collection of information during the patient's initial visit to the clinic and throughout their treatment journey Assist patients in completing necessary documentation at the clinic to prevent errors and ensure accurate information Manage the scheduling of patient appointments and maintain the patient's medical records Collect payments at the time of service delivery Monitor coverage for all funding sources to confirm approval Conduct data entry and billing tasks Coordinate courier services (incoming and outgoing) and manage incoming and outgoing emails, mail, and faxes Provide administrative support to the Care Coordinator Manager and Clinic DirectorRequired Qualifications & Core Competencies
High School Diploma, G.E.D. or equivalent qualification Exceptional customer service abilities and telephone manners Strong organizational skills with the ability to prioritize and manage multiple tasks Excellent communication (both verbal and written) and interpersonal skills Proficient computer and data entry capabilities Adaptable and flexible in response to change Experience in a Physiotherapy setting is an advantagept Health is committed to inclusivity and encourages applications from individuals with disabilities.
