About the Role
We are seeking a skilled Nurse Practitioner to join our team in a Regular Part-Time capacity. As a key member of our healthcare team, you will provide high-quality patient care, working collaboratively with patients, families, and healthcare professionals to deliver connected, accessible, and patient-centered care.
Key Responsibilities
- Provide holistic patient care based on advanced nursing and basic medical management knowledge and skills for a selected patient population.
- Liaise with patients, families, and healthcare providers to ensure seamless care coordination.
- Conduct medication reconciliation and review medication protocols with patients and caregivers.
- Identify patients requiring accelerated assessment and home care services, working with the Care Coordinator to facilitate the home assessment visit.
- Participate in establishing, maintaining, and monitoring standards for Case Management, including committee work and quality initiatives.
Requirements
- Membership in good standing with the College of Nurses of Ontario.
- Registered Nurse in the Extended Class (RN EC designation).
- Working knowledge of community resources and roles of healthcare professionals.
- Specialized education in palliative pain and symptom management (an asset).

- Pay Rate: Wages are in accordance with the collective agreement between Ontario Health atHome and OPSEU, with salaries ranging from $54.377/hr. to $63.161/hr.
