Key Responsibilities- Develop and maintain effective care plans for patients receiving community health services
- Provide nursing services to patients in their homes, including one-on-one care and virtual consultations
- Communicate and liaise with patients, family members, and other members of the interdisciplinary care team
- Perform other clinical duties as required

- PAID MILEAGE at $0.41 per/km PLUS travel time
- Flexible hours, autonomy over your schedule, and a variety of assignments

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing

- Exceptional interpersonal skills
- A valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- To protect our patients, we require a current vulnerable sector check and a clear background check
