Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team in Primary Care. As a member of our interprofessional team, you will provide compassionate patient care and work collaboratively with others to deliver high-quality healthcare services.

Job Summary:

The Licensed Practical Nurse will be responsible for providing clinical follow-up, administering medications, and performing assessments and care plans. You will work in a dynamic environment, collaborating with healthcare professionals to provide patient-centered care.

Responsibilities:
  • Provide clinical follow-up and care to patients in accordance with their primary care plan
  • Administer medications and perform assessments as required
  • Collaborate with healthcare professionals to develop and implement care plans
  • Work independently and as part of an interprofessional team to deliver patient-centered care
Requirements:
  • Graduation from a recognized program for Practical Nurses with one year of recent, related experience
  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant
  • Current, valid BC Driver's License
