Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our Addiction and Mental Health Outreach Team in Lethbridge, Alberta. As a key member of our interprofessional healthcare team, you will provide high-quality patient-centered care to individuals experiencing severe and persistent mental illness, addiction, and homelessness.

Key Responsibilities:
  • Provide consultative assessments and intakes for clients to determine physical and mental health stability
  • Complete thorough health assessments, treatment planning, management, and monitoring of both patient physical and mental health issues
  • Work collaboratively with community partner teams to provide care to individuals with unstable housing, socioeconomic factors, and substance use
  • Demonstrate a knowledgeable and caring clinical practice grounded in addiction and mental health nursing from a recovery-oriented perspective
Requirements:
  • Completion of an accredited practical nurse education program
  • Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Demonstrated ability to work independently and as part of an interprofessional team
Preferred Qualifications:
  • Experience working with vulnerable populations, including homeless individuals
  • Experience providing outreach care and working with clients with addiction and mental health issues
