Job Summary

We are seeking a highly skilled Ergotherapist to join our team in a Home Support Specialist role. As an Ergotherapist, you will work with patients in their homes to assess and address their functional abilities, develop and implement treatment plans, and provide education and support to patients and their families.

Key Responsibilities

  • Evaluate patients' functional abilities and develop treatment plans to address their needs
  • Provide one-on-one therapy sessions to patients in their homes
  • Collaborate with patients, families, and healthcare teams to develop and implement care plans
  • Conduct home assessments to identify and address safety and accessibility concerns
  • Develop and implement strategies to promote patient independence and autonomy
  • Participate in interdisciplinary team meetings to discuss patient care and develop care plans
  • Document patient progress and outcomes
  • Participate in quality improvement initiatives to enhance patient care and services

Requirements

  • Diploma in Ergotherapy or related field
  • Membership in the Order of Ergotherapists of Quebec
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
