Medical Care Coordinator

**Job Summary**
We are seeking a dedicated and compassionate Nurse Care Specialist to join our team at Renaissance Care. As a key member of our healthcare team, you will play a vital role in providing high-quality, person-centered care to our residents.
**Key Responsibilities**
- Lead a team of healthcare professionals in delivering exceptional care to residents, ensuring their medical, emotional, and social needs are met.
- Develop and implement personalized care plans, working closely with residents, families, and other healthcare professionals.
- Administer medications and treatments with precision and attention to detail, adhering to strict protocols and procedures.
- Collaborate with colleagues to ensure seamless care transitions and effective communication.
- Conduct regular assessments and evaluations to ensure residents' care plans are accurate and up-to-date.
**Requirements**
- Current registration with the Nursing and Midwifery Council (NMC).
- Strong knowledge of professional care standards and up-to-date clinical practices.
- Experience in a similar role, with a proven track record of leading a multi-disciplinary team.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and colleagues.
