Job Summary

Key Responsibilities
  • Diagnosis and Treatment: Diagnose and treat previously undiagnosed patients for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice.
  • Monitoring and Planning: Monitor ongoing care, order appropriate screening diagnostic investigations, interpret reports of investigations, and analyze information to monitor progress and plan treatment.
  • Patient Education and Support: Communicate with patients and families about health findings, diagnoses, priorities, outcomes, and prognoses; support and counsel patients in their responses to diseases, disorders, and conditions.
  • Collaboration and Consultation: Collaborate and consult with physicians or other healthcare and social service providers as appropriate to assess and diagnose patient status; develop and implement treatment plans.
  • Prescription and Medication Management: Prescribe drugs within statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
  • Leadership and Education: Assign work to other nursing and healthcare personnel, evaluate work, provide education and supervision as necessary, hire staff, and evaluate staff performance.
  • Research and Quality Improvement: Participate in research contributing to improved patient care and advances in nursing, health policy development, and population health.
  • Population Health Focus: Implement screening and health promotion activities for populations at risk.
  • Interdisciplinary Collaboration: Participate in interdisciplinary staff and nursing education through case presentations; mentoring; role modeling; and facilitating the exchange of knowledge in the classroom, the clinical setting, and the community.
  • Policies and Procedures: Develop, implement, and evaluate policies and procedures related to nursing, interdisciplinary care, and health system practices.
  • Additional Responsibilities: Act as most responsible provider and provide outreach to palliative, hospice, and long-term care when required.

To be successful in this role, you will require a strong background in nursing, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced healthcare environment.
