Palliative Care Nurse

Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse to join our Palliative Care team at Providence Health Care. As a Registered Nurse, you will play a vital role in providing high-quality patient care, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients and their families.
Responsibilities:- Assess, plan, implement, and evaluate patient care using evidence-based practice and clinical judgment.
- Develop and implement individualized care plans, incorporating patient and family-centered care principles.
- Provide education and support to patients and families, promoting health literacy and self-management skills.
- Collaborate with the interdisciplinary team to coordinate care, ensuring seamless transitions and optimal patient outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
- Current registration with the BC College of Nurses & Midwives as a practicing RN registrant.
- Ability to communicate effectively, both verbally and in writing.
- Knowledge of cardiopulmonary resuscitation (CPR) procedures and ability to perform them as required.
- Ability to work effectively in an interdisciplinary team environment.
- Strong analytical and decision-making skills, with the ability to prioritize and organize work.
