Job Summary

We are seeking an experienced Ergotherapist to join our team as a Home Support Specialist. As a key member of our interdisciplinary team, you will be responsible for providing occupational therapy services to clients in their homes, promoting independence and autonomy.

Key Responsibilities

  • Evaluate and assess clients' functional abilities and develop personalized treatment plans
  • Conduct home visits to assess clients' living environments and provide recommendations for adaptations and modifications
  • Collaborate with clients, families, and healthcare professionals to develop and implement plans for improving functional abilities and promoting independence
  • Provide education and training to clients and their families on strategies for maintaining functional abilities and preventing decline
  • Participate in interdisciplinary team meetings to discuss client care and develop plans for coordination of services
  • Document client progress and outcomes in a timely and accurate manner

Requirements

  • Diploma in Occupational Therapy or related field
  • Membership in the Order of Occupational Therapists of Quebec
  • Experience working with older adults and individuals with functional impairments
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
