We are seeking a highly motivated and dedicated Registered Nurse to join our team in Primary Care at Northern Health. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities in a primary care setting.
- Develop and implement care plans in collaboration with the interprofessional team, taking into account the patient's primary care plan and personal goals of care.
- Work effectively with patients, families, and healthcare teams to promote person-centered care and improve health outcomes.
- Participate in team huddles and other activities to promote communication, cooperation, and collaboration across disciplines.
- Contribute to the development and implementation of policies and protocols to ensure high-quality patient care.
- Three years of recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Experience developing and providing formal education to patients, families, and the general public.
- Driver's License.
- Ability to communicate effectively with patients, families, and healthcare teams using verbal, written, and computer communication means.
- Ability to integrate and evaluate pertinent data to problem-solve effectively.
- Ability to promote person-focused care that demonstrates care for and with people and their families/significant others within the context of their community.
